Comprehensive Continuum of Care Services:

Treatment and services are offered to individuals experiencing acute psychiatric disorders/illnesses who are in danger of harm to self, harm to others or are gravely disabled. In an attempt to provide services, at the least restrictive level of care, the organization office follows a programming service range which includes the following: inpatient hospitalization, partial hospitalization program (referred out), and intensive outpatient services (referred out). Admission to any level of care is indicated by the uniform application of the level of care criteria indicator. All levels of care are inherent in their ongoing assessment, treatment, discharge planning, therapy, and community/continuing care follow-through. The intensity of service provision depends upon admission criteria and the needs of the patient and the patient’s family/legally responsible person as applicable.

Service Development Department:

Our Service Development Representatives (SDRs) are active in community education and provide intake inquiry/pre-screenings/assessments (by qualified personnel) in community-based services and facilities, residential living environments, outpatient offices, and governmental agencies. SDRs are available for consultative and educational services. This is one way in which the organization feels it contributes to enriching community professionals and the general population with mental health prevention awareness. The organization continues positive relationships with community resources and networks with local hospitals, Aging and Adult Services, the LCSW/ LPC Board and Mental Health Services as well as other resources required by our patients to provide a seamless continuum of care and maintain positive collaborative relationships to improve overall community life

Central Intake Services:

Our Central Intake department serves as the primary point of contact for individuals seeking admission into our various levels of care. Trained intake professionals conduct comprehensive assessments to determine the most appropriate treatment options based on the individual’s unique needs and psychiatric conditions. This streamlined process ensures that all patients receive timely, compassionate, and confidential care while facilitating smooth transitions into the recommended program, whether it be inpatient, partial hospitalization, or outpatient services. Central Intake also coordinates with referral sources, families, and community agencies to provide a seamless entry into care, fostering continuity and enhancing the overall patient experience.

Adolescent Services:

Our adolescent services are specifically designed to address the unique mental health needs of teenagers, providing them with a safe and supportive environment to work through emotional, behavioral, or psychiatric challenges. Adolescents are treated by a multidisciplinary team that includes psychiatrists, therapists, and specialized staff who focus on family involvement and age-appropriate therapeutic interventions. Our adolescent program offers a continuum of care that includes comprehensive assessments, individualized treatment plans, individual and group therapy, family counseling, and educational support. Our objective is to help young people gain the coping skills they need to successfully navigate life challenges while improving overall emotional well-being and fostering long-term resilience.

Adult Services:

Our adult services are designed to provide comprehensive mental health care for individuals facing a variety of psychiatric conditions. We offer a range of evidence-based treatments tailored to meet the specific needs of adults dealing with mood disorders, anxiety, trauma, psychosis, and other behavioral health issues. Patients receive individualized care plans that may include medication management, psychotherapy, and holistic support services. Through a combination of group therapy, individual counseling, and skill-building sessions, we help adults develop healthier coping mechanisms, improve interpersonal relationships, and achieve greater emotional stability. Our focus is on empowering patients to regain control of their lives and reintegrate into their communities with confidence.

Senior & Geriatric Services:

Our senior & geriatric services provide specialized care for older adults experiencing age-related psychiatric and cognitive disorders, such as depression, anxiety, dementia, and other memory-related conditions. We understand the unique challenges that come with aging, and our interdisciplinary team is skilled in addressing the physical, emotional, and cognitive needs of this population. The program offers a structured, supportive environment where patients receive individualized treatment, including medical and psychiatric care, cognitive rehabilitation, and therapeutic activities designed to improve quality of life. Family involvement is encouraged, and discharge planning includes connections to community resources that help maintain a safe and fulfilling lifestyle for our senior patients.

Inpatient Services:

Our inpatient services offer a highly structured, secure environment for individuals experiencing acute psychiatric crises requiring the most intensive level of care. This setting provides 24-hour medical supervision, psychiatric care, and therapeutic interventions tailored to stabilize individuals who may be a danger to themselves or others, or who are unable to care for their basic needs due to mental health conditions. The inpatient program is designed to deliver individualized treatment plans that include medication management, crisis intervention, group and individual therapy, as well as discharge planning that connects patients with appropriate follow-up services. Our goal is to help patients regain stability and develop the tools necessary to thrive in the community upon discharge.

Intensive Outpatient Services:

Intensive services provide the least restrictive level of care for those patients needing more than traditional outpatient services as a means to receive treatment and ongoing support while being maintained in the community. For the continuity of care Freedom Behavioral will currently refer all patients that may require this needed service to the nearest community program.
